Healthmine – Healthy Connections (VAS gift reward card fulfillment)

TCHP has a new partnership with Healthmine, branded Healthy Connections for our members, for our VAS gift reward card fulfillment. Healthmine provides an online, interactive portal where members can actively participate in tracking their health actions and fulfill VAS gift cards. The VAS redemption will not be fully functional until approximately October 15, 2022, but members will still have a way to contact Healthmine to put in their requests starting September 1, 2022.

Please see information on how members can request their reward and create their Healthmine account:

  • All VAS fulfillment information can be found on the TCHP Healthy Rewards site (healthyrewardsprogram.org)
  • For all Healthmine fulfilled VAS (Prenatal, Postpartum, Cervical Cancer, Diabetes Management, Well-Child Check and Young Adult Wellness Visit), direct members to the Healthmine customer service number 866-475-1619 (TTY711)
  • Healthmine customer service agents can help members create their Healthmine account (if they have not already done so) and intake the member request for their VAS
  • Once the portal is live, the member will be able to request their reward through the portal
  • Healthmine is already live for the Health Risk Assessment – so please encourage members to complete this survey once they are registered for Healthmine

Next steps for providers: Providers should share this information with their staff and their TCHP plan members.
